Form No Longer Going to PayPal

my form @ http://www.armadillocon.org/membership.shtml working fine yesterday, suddenly, no longer redirects paypal upon submission.  somone can fill out , submit form, people not paying me money.  did change?

i tried out , re-directed paypal.  can delete submission.  see happening after submit?
