Alignment of text entered on forms

hi.  realized there adobe forum.  hoping can assist alignment issue.

 

have several pdf forms form site (www.singaporedoc.com) users can download.

however, whilst text area allows @ least 2-3 lines of text, text when first entered appears low down, missing first line.

can't figure out how solve this.

 

eg. if @ http://www.singaporedoc.com/index.php/orthopaedic-surgery/?filter_order=cd.name

there pdf link there.  under "clinical diagnosis" , "current medication" above problem.

i'm not seeing "clinical diagnosis" nor "current medication" on page. link pdf file directly if want help.

NIK SELECTIVE TOOL INTERMITTANT FAILURE.

ref: win7-64 /ps cs6 /google-nik collection.   after working "a time", select brush option in nik plugin, & when image "re-opens" in ps, plugin has selected ps brush tool, & can paint on effect i'd created, selective tool interface fails open, & there's no way apply or discard effect. at point ps "hung", waiting command either apply, or discard brush effect, can't done without selective tool interface; , way out, ctrl/alt/del close photoshop-- & lose work!  (results in ps appscrash result). the selective tool folder contained selectivepalette.8li file, along seltoolinst.cfg (instructional script tool??), , hdrefexpro2.jsx file. nik told me hdrefexpro2.jsx file did not belong there, & delete it, , sent me seltoolinst.cfg file replace existing one; , fixed problem short time! now works awhile, fails.---wont open either brush command, or automate. i check folder, & "something" has placed/re-placed deleted  hdrefex...
