Form Curser Jumps Around

i created multi-question form in dreamweaver.  when published server curser jumps first field in mozilla firefox when mouse used move through form (any field.) using tab works fine.  happens in mozilla browser.  ie works fine.  can me correct please?  following page holding form.  in advance.

the problem form caused unclosed <label> tag around today's date field.

 

incidentally, might want review  html5 form syntax since you're using html5 doc type.  far, you're not taking full advantage of html5 forms can do.
